## Question

Explain man-instrument system with its five objectives. What are the most indispensable system components required for man-instrument system to transmit an ECG and HR signals of a patient who is being medically evaluated to the better center after some serious accident at a remote place? Complete a schematic lay-out diagram showing the information flow between a mobile unit and a base unit.

## Short answer

A man-instrument system combines a human subject with instrumentation to measure, process, and present physiological data. Its five objectives are information gathering, diagnosis, evaluation, monitoring, and control/therapy. For remote ECG/HR telemetry, the mobile unit needs ECG electrodes, preamplifier, HR meter, modulator/transmitter, and battery; the base unit requires receiver, demodulator, amplifier, display/recorder, and voice link for physician instructions.

## Answer

The **man-instrument system** is the combination of a human subject (patient) and the instrumentation that measures, processes and presents data about the subject. The subject is the source of the signal and may also receive stimulus or therapy.

#### Components
```text
 Subject --> [Sensor/ --> [Signal --> [Display / --> Operator
 (man)        Transducer]  conditioning] Recording]   (doctor)
   ^                                        |
   |<-- [Stimulus/feedback/control] <-------+
        [Data storage & transmission]
```
Parts: subject, stimulus, transducer, signal conditioner, display or recorder, data storage and transmission, and control and feedback.

#### Five objectives
1. **Information gathering:** collecting data on the functioning of the body (research and basic study).
2. **Diagnosis:** finding the cause or disease from measured data (e.g. ECG for arrhythmia).
3. **Evaluation:** assessing the condition and the effect of treatment or a new device.
4. **Monitoring:** continuous watching of a patient's vital signs (ICU, operation theatre).
5. **Control (therapy):** using the measurement to control a device that treats the patient (pacemaker, ventilator, infusion pump).

#### Components needed for ECG and HR telemetry from a remote accident site
**Mobile unit (patient side):**
- Surface ECG electrodes (Ag/AgCl) and lead wires.
- ECG preamplifier with filter (0.05 to 100 Hz) and isolation.
- Heart-rate meter (R-wave detector and cardiotachometer).
- Modulator (FM or digital) and transmitter with antenna (radio, GSM or satellite link).
- Battery power supply; optionally a voice link to the paramedic.

**Base unit (hospital side):**
- Receiving antenna and receiver, demodulator.
- Amplifier, ECG monitor or chart recorder, heart-rate display, data storage.
- Voice link and a physician giving instructions back.

#### Schematic layout (information flow)
```text
 MOBILE UNIT (ambulance/site)       BASE UNIT (hospital)
 +--------------------------+       +-----------------------+
 | Patient                  |       | Receiver + antenna    |
 |  | electrodes            |       |  | demodulator        |
 |  v                       |       |  v                    |
 | ECG amp + filter         | radio |  Amplifier            |
 |  | ----> HR meter        | ----> |  | ECG display/       |
 |  v                       | link  |  |  recorder, HR      |
 | Modulator + transmitter  |       |  v                    |
 |  | antenna               | <---- | Doctor + voice/       |
 +--------------------------+ voice |  instructions         |
                                     +-----------------------+
```
The base unit sends back voice instructions so the paramedic can give treatment before arrival.

The answer's key points as multiple-choice questions, with the correct option and why.

1. What does a man-instrument system consist of?
   A. Only the electronic instrument that measures the signal
   B. The human subject together with the instrumentation that measures, processes and presents data about the subject
   C. A doctor and a hospital computer network
   D. A patient and a drug delivery schedule
   Answer: B. It is the combination of the subject (source of the signal, possibly receiving stimulus or therapy) and the instrumentation. (Key point: Man-instrument system)

2. Which of the following is NOT one of the five objectives of a man-instrument system?
   A. Information gathering
   B. Monitoring
   C. Control (therapy)
   D. Data encryption
   Answer: D. The five objectives are information gathering, diagnosis, evaluation, monitoring and control (therapy). (Key point: Five objectives)

3. Which chain correctly shows the signal path in the mobile unit for ECG and HR telemetry?
   A. Electrodes, ECG preamplifier with filter (HR meter in parallel), modulator and transmitter with antenna
   B. Antenna, demodulator, ECG monitor, electrodes
   C. Electrodes, chart recorder, demodulator, receiver
   D. Transmitter, electrodes, battery, display
   Answer: A. Patient-side signals go from electrodes to amplifier/filter, with an R-wave HR meter, then are modulated and transmitted. (Key point: Mobile unit components)

4. Why is a voice link from the base unit back to the mobile unit included?
   A. To increase the ECG bandwidth
   B. So the physician can give instructions and the paramedic can treat the patient before arrival
   C. To power the mobile unit remotely
   D. To calibrate the electrodes
   Answer: B. The base unit sends voice instructions back so treatment can start before the patient reaches hospital. (Key point: Base unit and voice link)
